Request Form for I-20

F-1 Students

This request form is used to request an updated I-20 for:
  • Change of Major
  • Change of Educational Level
  • Change of Funding
  • Lost or Stolen Document
  • Adding F-2 dependents
  • I-20 for a returning student who is still fully admitted to WSU
Please note that "Change of Funding", "Adding dependents", and "I-20 for returning students" will require updated financial information.
